this one happened in my early days, but it still sticks in my mind....
IG firing line vs. Ghazzgkull and his mateys. he had smashed a great hole through my line, and by the next turn he would surely get to grips with my tanks and guardsman, with the mighty horde behind him. *ahem narrative* Sgt Lupo and his vet squad were next to the gaping hole blasted apart by Ghazzgkull's wagons, and poured every ounce of fire into the warlord. he had declared his waaagh! so the plasma shots I threw at him were pointless AP wise. however some strait shots through the iron carapace of his mega armour reduced him to a single wound. Lupo wasn't gonna stand for that, and it was his fellow vets he thought of as he pulled out his laspistol and blasted a single beam of red light at the mighty Ork. the shot sailed past Ghazzgkull's grin of malice and hit him directly in his good eye. with a roar he collapsed to the ground. from there the IG collum blasted his remaining forces away. the battle was won thanks to a 7 point veteran sergeant slaying a 250 point warlord. a 3 to hit, a 6 to wound, and a invuln fail save of 1.
that day was a good day to be an IG player.

I had an ork battlewagon survive attacks from two tau flyers (stealth drone and a barracuda, and constant incoming fire from a riptide and a hammerhead. It was immobilized and had a weapon destroyed, but that's what mek's tools are for , and it survived the battle though I lost in the end.
Said battlewagon is named Da Fishstompa, and is easily my favorite unit/vehicle/model at this juncture.
In a battle with my Sisters against eldar, I had an exorcist and an eldar tank duke it our for several rounds. My missiles would fail to hit the tank, and I would Shield of Faith his tank's incoming fire. He had to assault me with some Deep Strike units to finally take out the Exorcist. I haven't thought of a name for that baby yet, bit I will.

My ridiculously flukey scout sergeant with a chainsword, bolt pistol and meltabombs:
-My scout squad watched a terminator squad get wiped by a daemon prince in two turns, thought "feth it", charged, survived five turns in combat and the now lone sergeant ran back a few inches due to failed morale, turned around and finished it with a bolt pistol round to the face.
-He once charged two feet of open ground in a game of apocalypse dodging shell fire, made it to the enemy's front lines, leapt a trench section, got 3 out of 4 of his men wiped on the way, climbed a wall, meltabombed a leman russ, the last man got killed by the second russ, and he blew that one as well.
-Went on a suicide run in a 5000 point apocalypse game against guard, survived 3 tanks, a command squad, psyker and two infantry squad's shooting (being the only unit in range), killed creed, kell and their command squad, got blown to pieces and managed to bring a primaris psyker down with him.
-Lysander surviving whole four turns on one wound against the entirety of a 2000 point eldar force after bad luck caused his army to be crushed and his reserves mishapped, assisted only by two devastators and an empty, weaponless rhino, all of which heroically sacrificing themselves to ensure their commander's safety until the end of the game.
-One legion of the damned multi-melta marine, who survived a mishap and four heavy bolter-toting long fangs, killed a hellhammer in one shot and survived the apocalyptic blast which claimed the lives of two full squads of space wolves and two vindicators.

Daring Dark Eldar Raider Rider
|
Fighting the Tau, I had a Raider full of Kabalite Warriors blown up. Only two of the ten survived, but they remained pinned for most of the game, enduring shots from Farsight and Riptide alike. My left flank was about to deteriorate due to Riptide pressure and Kroot were guarding the objective on that side, leaving me on the losing side. The Warriors remain pinned until turn 4 where one of the snapshots, hits a Riptide. The Riptide then fails his 2+ save and his FnP, losing his last wound. Farsight tries to finish off that Warrior, but he refuses to die, running up in the last turn to contest the enemy objective (also getting linebreaker), and giving me the game.
He became my new Archon shortly after.

Wolf lord with saga of the warrior born, thunder wolf, power axe and belt of russ went ape sh*t when his unit of thunder wolves was killed by massed imperial shooting, he killed I think around 60 guardsman alone and ripped through a punisher. He pretty much single handedly won me the game.
Another time my KoS who was grimoired and had corrosive breath and lash of dispair and demonic resilience killed 10 kablalites, an archon who put him on one wound, then killed 5 chaos terminators, a full wound nurgle lord with daemon weapon and 7 plague marines who were on an objective. Also single handedly won me the game.

1. Wazdakka managed to wreck a Landraider with 3 hull points left by scoring three glancing hits.
With his Dakkacannon.
2. Sisters v Blood angels. Some jump troops had routed my Retributor squad off of the quad gun but I still needed it to take down his stormraven which would come in next turn. MY BSS jumped out of the Rhino and even sacrficed shooting just to get into base contact with the quad gun. Fully knowing that they would be assaulted next turn and wiped out.
Storm raven comes on and the Quad gun intercepts and manages to bring it down. The guys inside, 5 Sanguinary guard died in the crash with only the Libarian surviving.
3. Saint Celestine taking down a Wraithknight, sure it was almost dead already but my HQ taking down the largest (I think) thing outside apocalypse will never be forgotten.

6th. my orks versus his Necrons Mission was The Relic. The nob from a trukk squad is the sole survivor of overwatch (due to a mis-interpretation of the Triarch stalkers rules, but it was my faulty logic) on turn 2, takes out the Triarch on 3, joins an assault on some warriors, takes them down as the sole survivor (he helped the remaining half of my other trukk mob) on 4, grabs the Relic on 5, games ends on six, with him in a trukk, controlling the Relic.
another one i like to tell is all the way back in 3rd, i had a lone Killa Kan obliterate 4 Blood Letters and a Blood Thirster in CC, over the course of 4 turns (we had agreed to play to a board wipe.) the thirster never got more than a "may not move or shoot next turn"

Few years back (possibly late 3rd Ed/early 4th Ed) I was played IG against Eldar and had a pair of snipers on a low hill.
Snipers ended up getting charged by a Eldar CC squad, survived without taking a scratch and thus forcing the Eldar to flee before cutting down and wiping out what was left of the squad.

My Nurgle Lord on Bike took on a Striking Scorpion Exarch and Karandras, managed to roll on the Boon Table, got the d3+1 extra results, and was sitting with Hammer of Wrath from his Gift of Mutation, Hatred (from his Warlord Trait), T7, and 2+ Armor save... ALMOST took down a WraithKnight... we were making save after save against each other, I then switched to my Power Axe, and he failed two saves and died (he had 1 on him from Karandras).
Back in 5th ed, I had a Sternguard Squad drop pod in with a Librarian, they nuked a Necron Warrior Unit, and then the entire army turned on to the unit and nuked them with the exception of the Librarian and 1 Sternguard. They get assaulted by a unit of Wraiths, and the Librarian takes down 2 but dies in the ensuing melee, and for the next three rounds the remaining Sternguard keeps living (the Wraith keeps whiffing) and finally kills the Wraith... only to be gunned down

An Enemy Fire Warrior squad did something pretty heroic in a game a while back, killing a tactical squad of mine + my librarian.
If you want to know what my models have done really well, a Sergeant of mine killed a Nurgle Sorcreror on Bike in a challenge. I like to think that Sergeant was awarded a medal and a pat on the back.
My Contemptor Mortis shot down a Heldrake with his Assault Cannons after the Missiles managed to live up to their name. It then went on to obliterate a few other things before going down to a melta bomb in close combat.
My paired Vindicators once destroyed a Land Raider and then everyone inside in one turn. Obliterating over 500pts in models in two shots always gives me a warm fuzzy feeling.
One of my Rhinos once killed a Beast of Nurgle that was contesting an objective.

if Fantasy counts in here, and 6th edition at that.....
I was outside a dwarf fortress when a gyrocoptor landed next to my unit of crossbows and fired, leaving one guy left, who passed all his checks etc.
He then charged into the gyrocopter, downing it by shoving his crossbow in the mechanism and sending it spiralling into my steam tank, which was about to attempt to breach the gates of the fortress, the gyrocopter crashing into the steam tank made it explode, killing 3 spearmen and destroying a siege tower.
because I couldn't get into the fortress after this I lost, as I just could not get up the walls,
No one has seen the crossbowman since, though it is reported to have said 'Ooops'.
He really was heroic, destroying a gyrocopter with a crossbow, but the result wasn't the intended one!

Infiltrating Broodlord

Lets see, another recent game. It was 500pts or so of my nids vs a cron airforce. I think it was 2-3 aircraft. Anyhow, I run some devilgants into a corpse of trees for cover, and they lost synapse the turn before. They gtg for some epic cover saves and the real fight begins. My other units are quickly destroyed turns 2-4, and the crons hop out onto their objectives. The birds in the sky go after the gants, and after a round of shooting, a little less than half the small group of maybe 11 or so gants are dead. The gants open up with some pointless retaliatory fire and scratch the paint on one of the fliers. Next round comes, and the birds open up again. One gant survives. The gant opens fire again, and his punative fire scratches the paint, yet again. Next turn comes around, and the gant survives. The dice are rolled, and the game goes on. Several crons hop off objectives to get in range. Pretty much everything opens up at this point, and the gant lives again. The gant takes one last shot at the same aircraft it fired at the turn before, and scratches the paint yet again. The game ends after that. One little devilgant who couldn't do any harm, managed to give the crons a defiant end, and skulks off into the night, the lone survivor.

6 Assault marines + a SM captain charged my lone draigotau commander (S5, T5, 2+/4++, FnP, fusion blaster, cyclic ion blaster, onager gauntlet)
Killed 2 on overwatch, they caused 1 wound, I killed another 2, then next turn punched the SM commander in the face with the gauntlet's str 10 ap 1 hit, and finally grinded out the 2 remaining marines.
The above scenario has happened a few times before under similar circumstances. People just don't expect a tau commander to have that kind of resilience and CC/ranged power for 90 points less than draigo

Stoic Grail Knight
|
Eldar vs Grey Knights, old Eldar Book, I think I had some Vostroyan Allies as well. Getting towards the end of the game, so the Forces are starting to look pretty thin. I've got a Fire Dragon squad which just fired into the flank of a Paladin Squad, but my opponent doesn't want to waste hundreds of points of Paladins killing 80 points of Dragons when there are bigger fish to fry. Enter Dread Knight, hes down to a single wound after various shooting from my army, and my opponent sends him to deal with the Dragons. Incinerator Fires, leaving one alive, and the Dread Knight charges. the Lone Fire Dragon fires his overwatch and.... misses. Now in close combat the Aspect Warrior decides to forgoe his normal attack at initiative 5... and the Dread Knight misses horribly, after the greatsword re-rolls he lands 2 blows, and rolls snake eyes to wound! At this point the Dragon rolls a hit (melta bomb connects), rolls a wound (melta bomb wounds), Dread Knight rolls his save and goes down. Lone Fire Dragon kills Dread Knight with melta bomb in close combat. He was promoted to Exarch for that little stunt. ____________________ On another note, a running joke at my FLGS are my Slaanesh Lord's rolls on the Chaos Boon table. A typical example of this came up in a game against my Friend's Salamanders. Falcon Punch Vulcan in the face in an epic duel... Unworthy Offering Tear apart a generic Sergeant without a second thought however... Dark Apothesis! Oh Chaos...

After the second time, the Necrons lost leadership, the Wolf Priest caught them in the sweep and killed the Immortals.
