4th edition 500 pt. escalation league game.
I ran IG and he had a Demon Bomb with an Exalted Champ with Wings/Infiltrate (1st turn charge capable). He goes first, assaults an Infantry squad. Kills 3 and the rest manage to wound him. My Tech Priest assaults on my 1st turn and scares him with his power weapon and servo-arm. Champ attacks him and kills him. The last of the Infantry squad kill the Champ! No way the Demons can be summoned. First Turn WIN in CLOSE COMBAT for the IG!

Win: Charged a heavy weapons squad with a single wrack, did 2 wounds, won the combat and swept them.
Win: Killed the sanguinor in one round of shooting from 5 kabalite warriors with splinter rifles.
Fail: playing a proxy GK army against nids just to try it out, had Coteaz and a tooled up henchmen squad in a razorback, put its back against the wall to protect rear armour, got assaulted from both sides, vehicle blew up and he and his squad died because all the access points were blocked.

Wins:

Massacre moving through a whole Tau army in 4th ed with nothing but a Warboss.

CCS about to be charged by 2 LC Terminators. The CCS is the only unit contesting the objective. It is the last assault phase, the Termies are within 2". Overwatch my rapid firing plasgun and my officers plasma pistol. 2 hits 2 kills. Made all the more sweeter by the 3rd shot overheating and killing my own plasmagunner.

CCS commander had bodyguard gunned down by 3 man stealth team. Assaults them next turn, pimp smacking thm into a pulp with his powerfist. With 1 wound left he assaults a squad of 10 marines, and over the next 2 turns flicks 7 of them out of existence (including a PF sergeant). The remaining 3 run from combat, rapid fire him to death.

Losses:

Big Mek kills himself EVERY GAME with his own KMB...no exceptions. I kid you not, he's never got past turn 3.

Full 3 wound captain within 2" of a Tau FW squad of 12, gets overwatched to death.

Losing 30 Orks to a 5 man Grey Knight Termies squad.

Normally this should be impossible since Draigo is fearless. That said, if they pulled off the malediction that removes the effect of fearless (which just adds to how ridiculous this is, given GK's anti-psychic defense), I could totally see this happening.

So my store is doing this map campaign. 4 imperial fleets decide to attack craftworld kaelor. 2 CSM fleets decide to aid the poor Eldar. 6000 pts of mixed imperials, vs 3000 pts of Eldar/CSM. Due to time constraints, game was called after turn 2. Had it played out, the imperium would have been annihilated.

Greebo had spent an irritating two minutes in that box. Technically, a cat locked in a box may be alive or it may be dead. You never know until you look. In fact, the mere act of opening the box will determine the state of the cat, although in this case there were three determinate states the cat could be in: these being Alive, Dead, and Bloody Furious.
Orks always ride in single file to hide their strength and numbers.

My best win was Black Templar (me) VS Necrons when my standard tactical marine was versing Vargard Obyron who was on his last wound and my guy head butts him and Vargard Obyron failed his armour save and dies giving me slay the warlord and winning me the match

My worst lose was I was playing Tau VS Grey Knights, and it was big guns never tyre and my hammer head move from an objective to kill the enemy warlord, and this was turn 5 so I was hoping there would be another turn, when I failed killing his warlord and the game ended costing me the match, and he had his warlord left with 5 guys remaining, and this was a 2k match.

This is a brick in my wall of hatred for 2nd edition.

Was playing Eldar against a Tau in 5th edition. I don't remember exactly what had happened but a squad of pathfinders with a battle suit HQ managed to push deep into my lines. He had the ability which gave his unit next to him a 4+ cover save. I fired everything I had into him and he saved them all. I charged 6 guardian jet bikes into the squad into his pathfinders. I lost 3 jet bikes and lost my initiative test and was swept. Then I charged with 15 guardian defenders after they failed to kill the pathfinders. They failed to deal a wound, lost combat and got swept. I used my Dark reapers to shoot at them. They made all their cover saves. The return fire from the pathfinders caused to the Dark Reapers to go below half strength and run off the board. This was without the help of Markerlights.

Another game was against Tyranids the day before their new codex came out. Even with the Eldar codex upgrade into BS4, I have never rolled more 1s and 2s in my whole life. Wave Serpents died to Hive Tyrants strength 8 blast. Malantai came in next to destroyed wave serpent with Dire Avengers: guess who rolls a 15 on his ability? Farseer on Jetbike with Mantle of the Laughting God died to shooting gaunts. 3 turns in and my Crimson Hunter couldn't even come in. Avatar of Khaine died to a Carnifex in close combat. 5 attacks, couldn't land a single hit. By the end of Turn 3, out of a 1500 point army I was down to 3 guardian jetbikes and 2 Fire Prisms.

A good game I had in 40K (sort of) was in BFG (I know its not 40K but its my only winning moment). Eldar launched torpedoes at a Necron Tombship. Out of 14 torpedoes, 7 of them were 6s. There were 2 other witnesses and about 2 minutes of silence.

Two SAG meks with a shooty heavy army camped out on a skyshield. Tons off orks packed into that thing, and the enemy hadn't brought much in the way of templates/blasts. (space wolves)

Game was going quite well, until turn 3.

I decide to fire both my SAG meks at his wolfguard.

1,1 on both. Both explode max distance, wiping the skyshield clear of almost my entire army. Just a few startled grots left to face down some severely horked off wolf guard termies who had just lost their ride..

Ooh, just remembered a win moment. In 5th edition Yriel destroyed a monolith with his spear.
Then an epic moment, not sure if its win or fail. Yrield confronts the neuron lord. He throws his spear: rolls a 1. But his BS was 6 so he could reroll: rolls a 6. Proceeds to wound: rolls a 1. We imagined it to be a moviesque moment. Yriel throws his spear and the neuron lord tilts his head to the side to dodge it. The spear turns around and aims for the back of the head. Then the neuron lord tilts his head to the other side and avoids the spear completely.

Huge win:

Me playing against my friend right after the new Tyranid codex came out. He took tons of genestealers and was planning on using the broodlord psychic power to make my Tau line all pinned so he could close the distance and charge the next turn (make a LD test at -2 penalty. If failed you go pinned next turn. He cast about 5 copies of that at various units in my gunline).

So, I had no idea he could do that and it terrified me. But the Greater Good kicked in for me. I swear I haven't passed a single deny the witch since 6th edition dropped (seriously, I can't remember doing it one time before this game).

I passed 3 out of the 4 deny the witch rolls I had to make that turn (and I passed the leadership test on the 4th one). Yes, I rolled three 6s out of 4 rolls.

That one made my jaw drop...

Huge loss.
It was my 2000 points Imperial Fists Vs Biker nob army with pain boyz.
I set up gun line with troops and had 3 Missile devastator squads. He stole Initiative and his biker nobs got across the board so quick with turbo boosting, I fired 230 bolter shots with re-rolls and landed 94 wounds with the missiles over the course of 3 turns. I managed to kill 1 Biker nob, 1 biker boy, and put 1 wound on a biker Warboss. It was a disaster. between his cover saves and FnP rolls, I couldn't do anything that game, the dice gods really hated me that game, Only thing to do was just laugh and smile. only reason I didn't get tabled was Lysander and escort finally deep struck in at the end of turn 5, and then the game didn't continue.

Best Moment: Chaos Lord with axe of blind fury butchering Maugan Ra in one assault phase.
Worst: Same Chaos Lord then killing himself with his own daemon weapon in the next turn!

Also back in 2nd edition, Abbadon hacked his way through 3 mega armoured nobs, 3 ogryns (back when orks could use them) and 10 grots only to get held up by makari and his 2+ unmodified save!

We played the combat rules wrong as we thought that a follow up meant you could attack again in the same turn, but it was funny!

Also, in that game, my chaos rhino ran over and killed ghazghkull thraka!

I've got two in a recent game, 6th Edition CSM vs. Tyranids, 2k points.

Win: Chaos Terminator Lord and his Chaos Terminators retinue. Tanking ungodly amount of shots coming from a large group of Termagants, an Exocrine, 2 Dakkafexes and a Flyrant (not all of them at the same time, though) with the power of 2+/5++ and FnP, they proceed to wipe out Hormagaunts holding a Relic and destroy the Termagants with shooting and the following charge. The Terminator Lord (who notably had Skull of Ker'ngar, and SoC I took to even out the pts. value) said "Nope." every time a brood of Zoanthropes and the Flyrant targeted his squad with psychic powers and killed said Flyrant in two phases of CC without suffering a single wound. Dayum.

Fail: The same game. I failed miserably with 5/7 total Blastmaster Single Frequency shots, scattering so far it did not wound anything.

Well there was my PK Nob who killed a Daemon Prince...

Then there was yesterday during the start of and Apoc game, I was playin DA. Las Cannon Devs were being used for Herosniping. I killed 3 Biker Nobz Exploded a Looted Wagon, Killed a Killa Kan, my custom Snikrot. 3 Basic nobz and put a wound on a Warboss. I rolled at least 80% 6es on those rolls.

Playing in a small 1k tournament. Final game for first place was my Thousand Sons vs Eldar.

I was getting annihilated, needless to say. I had been able to take my opponent off of all his objectives by the end of 4th turn. On turn 5, he had wiped me out to all but a Forgefiend and a lone Thousand Son.

With only two models remaining, I moved over to the closest objective, using the Forgefiend to block LOS to my lone TS from a full squad of Rangers and a full Dark Reapers squad.

His start of Turn 6, he opens up on the Forgefiend, and destroys it, causing it to explode. I make my save on the Rubric Marine. Then, the Rangers open up on the Thousand Son, causing only one wound. I pick up my dice, and rolled it... a 4! I made my armor save! My part of turn 6, I roll my only shot I had, and ended my turn.

My opponent rolls to see if the game ends, and it did. Due to me having the only objective, and higher kill points, I won the tournament with a single marine standing on the board!

Okay, I played CSM (3.5 ed codex) in an RTT long ago.
I have fielded 2 Lieutenants on bikes, 5x6 Daemonettes, 6 Daemonettes on steeds, 3 Obliterators, and 1 Defiler.
The Lieutenants had 9 minor powers each to grant Siren which said that the carrier (no psychic test necessary) cannot be shot or assaulted. I used the Lieutenants to summon the Daemonettes which were able to charge upon arrival. My enemies couldn't do anything against this army. My opponents got nuts. I won all the game but didn't play this army again. Death to the false Emperor!

Eye of Terror

Win: Abaddon & 1 squad of plasma chosen in a Land Raider versus a 2,000 point space wolves army. My army was much bigger, but Abaddon and his lackeys did 90% of the offence. Almost tabled them.

Loss: Tau versus noise marines and bikers. Happened right after 6th edition started, just got demolished in the second turn. Did not cause any casualties with my CSMs.

Loss - I once rolled 14 1s in a row. Effectively it killed my Chaos Sorc, my Spawn, and my Noise Marines did pretty much no damage.

I needed those things to work. At least some of it.

Win - 6 snap shotting lascannon shots from Vendetta Gunships, all hitting the enemy HQ, and taking him out and scoring me the VP I needed.

Win - Had a broken squad of 3 guardsmen w/ lasguns deal the final wound to a Daemon Prince (4+ to hit, 6 to wound, 3+ save with re-rollable 1's, rolls a 2, lol). Proceeded to dominate the rest of the game.

Loss - BA Dreadnought with Blood Lance took out 2 of my Leman Russes out of a drop pod on turn 1, and it didn't go well after that.

My best fail was back in 5th, My Alpha Legion vs Guard.
I wrecked face, basically tabled my opponent by turn 4. All he had left was Commisar Yarrick, I had him surrounded with 2 depleted CSM squads, 1 small squad of Beserkers, a wounded Chaos Lord and a Vindicator. I was about to give the order to fire and shred him into pieces when I decided it would be more sporting to rip apart this imperial hero in combat. Charged everything in...My Lord managed to kill himself with his Daemon sword, everything else failed to wound, Yarrick then wounds with all his attacks on the beserkers, the rest die to Fearless, the CSMs flee, one off the board, the other was too under strength to rally. He then charges my Vindicator and wrecks it. I was speechless. It was like a typical action movie where the villain is about to kill the hero but then tells him his secret plan and then gets his ass kicked due to hubris. This is why I will always take playing for fun over playing to win, any day.

Best moment: a ten man assault squad gets shot to death leaving only the sergeant left against 9 Necron warriors. Sarge has a powerfist and combat shield. Charges Necrons. Proceeds to hold out for three turns in CC by himself making every save and hitting and wounding every time.

Biggest fail: Brother Corbulo fails armor save against an orc assault. Then I roll a 1 for his FNP. Next attack fail armor save then roll a 1 for FNP so I take the Far Seeing Eye ability and reroll the FNP...I roll another 1. Facepalm. Emprah really wanted him to die lol!

Epic Fail: I rolled 14 's on 14 dice with my orks one time. Getting that is 78364164096 to 1.

Minor Fail: Bought DV, first game with the DA, first turn, PC guy blows himself up.

Win: made an ork nob with a combi rokkit. First game with him, his squad (10 of em) get in range of a predator, only his rokkit can hurt it. So, rolled one die, he hits, pens, blows the turret off it, making it pretty much useless for the rest of the game (5 ed BTW)

I would have loved to have watched that game.

Fail: Had a Mantle Farseer roll box cars for his psychic test on his last warpcharge and had to take the wound. He then moved and proceded to fail a dangerous terrain test and his armour save, rolling a 1 for both. Moved Flat out on that same turn, and you guessed it, two more 1's. Dead Farseer

Fail: On turn 1, my full squadron of dual Scatter Laser War Walkers fires upon a unit of Grey Hunters that podded in. Squad is reduced to 2 models so I charge them in for an easy clean-up.
