I did a search but couldn't find this under "Daemon weapon specialist weapon" or "Daemon weapon close combat weapon."

A Khorne lord on a Jugger with Axe of Blind Fury will get, if I get this right...

3 attacks base, +2 for rage, +1 for the Jugger, + D6 for the Daemon weapon. Do you get an additional +1 for having a Daemon Weapon & Bolt Pistol (i.e., pistol and CCW)?

Daemon Weapon is just a rule for the weapons, they still have Melee in their profile and are not two handed. So yes you get the extra attack.

Yep. Up to 13 attacks on the charge...
