At least as early as 4th edition(2007) Mephiston is mentioned as being Calistarius, and Calistarius did not appear in Space Hulk until 3rd edition(2009).

IIRC He fell to the red thirst at some siege and got crushed under an exploding gateway. He succumbed to the black rage whilst trapped and suffered through madness for insert number of days here. Whilst trapped Sanguinius visited him in visions telling him that he must resist the black rage to show his brothers that it could be beaten. Eventually he beat the rage and unlocked his true potential allowing him to lift the rubble from him. At this point he ceased to be Calistarius and became Mephiston, the reason he is so 'ard is because he still suffers from the black rage he merely suppressed it through extreme force of will and therefore benefits from the increased strength and toughness it offers.

The interesting thing about Mephiston is that he beat the black rage and obviously has some huge benefits from it, yet is it a unique case or would all blood angels benefit from falling to it if they could overcome it?

Then we link it to other chapters, or marines from other legions, blood angels are not physiologically 'THAT' different to other marines other than a few legion specific quirks here and there. So, in theory, it isn't beyond belief that marines from other chapters could 'ascend' as such, yet none have managed it as of yet. The madness that the black rage brings wouldn't alone account for the physiologically changes that must occur, such as the massively reduced speed, strength and toughness, it must be a catalyst to unlock these changes already potentially present.

Pretty scary that it is possible that all marines could become that strong and tough... And obviously, this crosses over to chaos marines as well.

Imagine a marine he succumbs to the black rage, cures himself in the vain of mephiston and THEN falls to chaos and ascends to Daemon hood eventually.

That's of course if the some theories on mephiston are not true of course, one such being that his increase to attributes is because he has become a daemon prince of sorts, in which case the changes would be explained.

I assume they all could benefit, yet most aren't strong willed enough.

The black rage is largely a result of the Blood Angels psychic link to their primarch, they gain their strength from the black rage because they have visions of Sanguinius aboard the Vengeant Spirit during the assault on Horus. They relive the primarch's final hours and gain a measure of his speed, strength and resilience.

It probably wouldn't transfer to other marines from different geneseed.
