gonna second FITZZZ on this one, think 'the fly' and go with spawn + terminator lord bits to make it suitable pestilent.

Or on another tangent, what about creating a terminator armoured skaven plague monk? big ratty head poking out of suitable decayed and rusty armor with plague censer?

I once saw a nurgle aspiring champ shoulder being used as a head, and it looks quite cool.
