Valtiel from Silent Hill 3.

Though you never directly confront him, and he isn't very imposing, the fact that he's subtly pulling the strings and controlling most of the occurances throughout the game makes him one of the most terrifying creatures ever conceived.

That, and his design in itself is incredibly unnerving.
