All after dodging and dismantling our heavy weapons guy, our troll and me (a physical adept). We all whiff and the face strolls in and drops him with one shot.

GW: Years ago, was playing my BA vs a real jerk of an IG player. Just a very angry guy, letting whatever had curb-jobbed his happiness bleed all over his interpersonal interactions. Well, he takes a battle cannon shot at the barrel of my Chaplain's plasma pistol that is poking around a building (3rd ed). Scatters the template over the Chaplain and my 7 man DC. Calls all his friends over to watch him roll the dice, laughing like a belligerant child (he was mid-20's). Rolled 8 ones. Was heckled nigh unto crying.
Non GW: Dad challenges me to a game of chess (I was about 30 at the time). Dad fancies himself a pretty good chess player, and he was an arrogant so-and-so about it but also very correct, he was and is. Dad falls for a loosely disguised Queens Gambit and promptly gets harassed to death by all my cousins about the big bad chess master falling for the simplest, most well known gambit in chess.

Yeah I know I know. ;p But our GM is pretty adamant about keeping it hard on us. Despite the 8-12 damage shuriken, we actually came out of it in pretty good shape.

And for general RPGing, in this case Shadowrun from last night. A quick note on Shadowrun's rules for those unfamiliar, everything is based on d6 rolls. 5-6 are "hits" and 1s are "glitches". If you roll more glitches than hits, it is just a small fumble, but if you roll only glitches and no hits, it is a massive, critical fumble. If this happens, a character can spend a point of Edge (a stat that is pretty much used for big, important rolls) to re-roll it.
Now that you are briefed, here is the situation:
My character wants to use his gymnastics skill to vault over a desk and attack a bad guy taking cover behind it. All told, I roll 8 dice for gymnastics tests, and a vault like that requires 2 hits at most. So a pretty reasonable roll, right?
Roll 1: 1, 1, 1, 1, 1, 1, 2, 4
Erk. Well, no worries, I can use a point of Edge and...
Re-roll: 1, 1, 1, 1, 2, 2, 3, 4

Lol, so basically your character completely missed the table, and fell over? XD

Ive had one or two.
1 At my local GW we have this massive Apoc game once a month. during this game im the grand commander for my side(and thus am also an objective marker but more on that later)
Anyway my side has a Shadow sword on it. we get first turn. The player whom owns the Shadow sword fires it and kills a few orks(turned out later he had only used the large blast template rather than the 5" blast). One of our opponents then precedes to snipe it with his devestator squads lascannon. hits and then  all in a row. Its bye bye Shadow sword and half our force. We still somehow manage to get a draw(actualy it would have been a win but the shop staff called it).
2 Same game. My small force of Khantonians( IG) consisting of the contents of the fatpack box + a lord commissar a master of ordinance and a Grenadiers squad as well as a regimental command squad a 4 man sniper squad a 6 man(?) ogryn squad a special weapons squad and a 7 man stormtrooper squad hold off over twice there number in models of darkangels space marines even when flanked by some eldar and orks. By the time they actualy overrun my position theres only 5 of them left and my commander and lord commissar have gotten out anyway(plus i still have one lascannon left).

One day a few years back, while chatting with the clerk at my FLGS, a girl walked in and asked if we would like to see her new tatoo. We said sure. So she took off her shirt, braless, turned around and her back was nearly covered by an incredible Aztec Sundial. We made a fuss over how cool it looked for a few minutes, then she thanked us, put her shirt back on and left.
Clerk turned and said "did that just happen?"

Got a couple.
First off. We had a enemy warhound titan run into the middle of our apoc game. My baneblade steel legion came in, and nuked it. Literally. Apocalyptic explosion.
That ripped the heart from our guys force, Then it came on again (reinforcements stratagem) and blew up a baneblane. Then managed to crit it, apocalyptic explosion, then blew itself up into a....you guessed it. Apocalyptic explosion.
By then it was game over. A draw. But very close.

My Treeman was stuck in combat with the Giant (it was long time ago and I don't remember which edition it was). It was not the first round of combat, so everything went by initiative (which is 1 for Treeman), so Giant keeps using his special attacks first. For three turns Giant was using one dumb special attack - don't remember the title - but in the essence he was yelling or breathing badly, Treeman was making his saves or whatnot, but with that special attack the combat phase for them was over - I could not hit him back. Then, O&G player finally managed to roll for a different special attack - it was something nasty with no armor saves and lots of wounds UNLESS Treeman makes his initiative test. Which means on 2+ he is as sure as dead (nice ending after staring at Giant and doing nothing). So I roll a "1", special attack fails and I finally can hit the Giant, kill him, he falls over and takes lotsa orksies with him!

My miracle moment so to speak, last eidition Tau Sniper Drone vs Ven Dread.
closing turns of the game trying to kill whatever I can at this point, sniper drone hits, glances, and rolls a 6, dead dread, happy me. My "friend" (friend = TFG WAAC player) smirks at me and points out the reroll, low and behold another 6, as you can imagine a lot less smirking from him anyway.

Well, the 6-glitch roll was going to me be overshooting it and going face first into the wall. But with only 4 glitches on the re-roll, we determined that I caught my foot on the desk, and basically swung forward on my foot and levered myself into the ground.

I was playing my Draigowing list at a 2k FLGS tournament. It consisted of 2x10 diversified paladins, libby, Draigo, and a dreadknight. After securing a massacre in the first round I was feeling pretty good, even though I would be facing what was probably the toughest list for me on a tough table in the second round. The mission was capture and control, and the terrain was very sparse in the middle of the table. My opponent brought marines with tons of melta, so I knew that I needed to utilize cover or I would not do very well.
I decided to deploy my main unit of paladins with libby and Draigo in a large piece of terrain on my side of the table, and hold the other paladin squad and DK in reserve to deep strike onto my opponent's side and run into cover.
Second round came and my dreadknight scattered poorly and put back into reserve. Third round arrives, the DK scatters again and is lost in the warp. The paladins also come on this round and do the exact same thing, scatter badly and end up lost in the warp. So here I am, with over half of my army lost in the warp and really no way to get across the table to contest his objective.
My opponent was unable to move me off of my objective, but was able to fly up a land speeder and contest in the final turn, leaving me with a major loss.

Had a 1500 point game of World Eaters vs Ultra Marines. I assaulted my Khorne Berzerkers into his Sternguard squad that included his Chapter Master. The Berzerkers kill all but one Sterguard and the Skull Champion proceeded to instant death his Chapter Master with a Power Fist. Following this, he wasn't too pleased so he assaulted his squad of 5 Terminators with Power Fists and his tact squad of 10 marines into my remaining 8 Khorne Berzerkers. I had all of my Berzerkers try to kill the Terminators (Killed maybe one with I4) and lost 3 to the tact marines he assaulted in. The Terminators mopped up the other 4 with their Power Fists, leaving the Skull Champion. This Skull Champion would proceed to kill that squad of 5 Terminators, that final Sterngaurd from the initial assault, and 9 out of 10 tact marine that he assaulted in. This only happened because every model he had either failed to hit (Terminators excuse), or hit and wounded and he just made the 3+ armor save like a boss. He then proceeded to be elevated to a Daemon Prince following this as he killed over 50% of his list alone.

I was watching a game between CSM and IG a while ago at my local GW and the IG player is getting annihilated. There is one guardsman and a couple chimeras left on his side. In CSM shooting both chimeras get wrecked, and then the guardsman gets assaulted by a demon prince with only 1 wound(you know where this is going). The demon prince completely whiffs rolling 1's on all to wound, then the guardsman attacks back and kills the demon prince, the entire store was laughing for about 3 minutes after that. Then the guy got shot up by a bunch of termies after that, but had earned his spot in history.

In a 1000 point game it was my Eldar & Ork Ally vs CSM & Tau Ally. My Fortune'd Farseer and 6 Warlocks got assaulted by ~30 Bezerkers and a Khorne Lord with a Daemon Weapon all at once in the top of turn 2. I manage to roll/reroll all of my saves so well that I only lost a single Warlock until Turn 4 and end up killing all the Bezerkers but the Lord. The Lord was able to make B2B with my Farseer (since the Warlock that got killed left a hole straight to him) and the Lord kills him, with the Warlocks exacting vengeance by taking down the Lord afterwards. The Warlocks than proceed to make a  for their fleet run, and catch a group of 10 Fire Warriors holding the enemy objective, killing all but 2 and routing them. Getting them off the objective won the game for us!  I have never been so proud of my Seer Council, and I bring them much more frequently due to their performance in that battle.
Another time would be in a 3000 point 3v3 game where I was playing my IG. One of the enemies was Tyranid and brought the Doom of Malan'tai and dumped it into my Guardsmen causing mass death. I had an outflanking squad of Penial Legion come in as Psychopaths by the monster. The Doom of Malan'tai rips 9 souls from the squad, leaving only the Warden left but he saves leadership and holds. Everything in my army shoots at the monster, and it is left with 1 wound. The Warden is within assault range and charges the beast alone, managing to single handedly land the last wound and kill it. He lives till the end of the game, being also the survivor of a 21 Guardsmen (the 1 being him) + a Commissar assault into 6 Crisis Suits. Out of the assault, 6 Guardsmen and the Commissar live, one of the Guardsmen being the Warden. He's now been upgrade to my permanent PCS commander for his heroism, and he is also the only person (besides my CCS Commander, who has a white and red jacket) in my army that has a black uniform instead of green.
The last one I can recalled happened the Sunday before last with my Black Templars. It was 2v2 2000 point game, with my IG/ BT vs Chaos/Orks. The Chaos had brought a Thousand Sons oriented army which was perfect against my Black Tide BT list. As I was advancing up the field (taking horrific casualties) he got out with 10 Thousand Sons and went flat out with the Rhino they were in (actually landing pretty close to the SB squad). The Thousand Sons lay into my 8 Sword Brethren/Marshall/ EC squad, out of which only both Commanders and a single Sword Brethren lived (wielding a PF). Come my turn, my Marshall/ EC join a Crusader squad and attack some Orks. The lone PF Sword Brethren moves towards the Rhino and shoots his Bolt Pistol, and hits.  for penetration,  on the damage chart thus immobilizing it! He assaults it and pops the Rhino!  He truly lived up to his Sword Brethren title. He promptly got mowed down by a different squad of 8 Thousand Sons the next turn by the angry Chaos player, but he died valiantly.

So many come to mind, but a few only stick out clearly as if they happened five minutes ago.
1. Ultramarines vs Mentor Legion (Me)
The battle was coming to a close, and I had a few combat squads of sternguard with my lib and kantor holding it down and objective while none of the others were claimed. On his side, a brick of 8 TH SS termies with Strike and Vulkan are closing in on the remains of my army but then suddenly I have one shot to make it count. My lib casts null zone and it passes and then Kantor summons down an orbital strike, it does not scatter and the template hits them all. Needless to say, none of them live and my rag tag mentor legion scraps by a win.

This one happened last week.
Ghaz has a single wound left and is about to charge my last troop choice sitting on an objective. Before he does so he finishes his shooting phase by shooting an empty Chimera. Pens, Explodes it. We didn't both rolling for size but on a whim we thought why not. Got a 6 and Ghaz is exactly 6" away from the explosion. So close we decided to roll to see if he's actually in. I roll a 4 so he's in. 6 to wound. He rolls a 1 on his armor save loosing his last wound. That random decision to shoot a Chimera took him from winning the game to tieing it. Woot.

I have two stories, one that was really good for me, and one that was really not so good.
First story: I was playing vanilla Marines vs. Eldar and by the end of the last turn of the game the only guys left on the table were my Scout Sgt. and his Avatar, it was his turn, and he was in charge range. He charges my Scout, and fails to hit. I roll my two attacks (Bolt Pistol and CCW) and roll one 6, roll another 6 to wound, he then rolls a 1 for his armor save, and was on his last wound. Thus ending the game with my Scout being the only model left on the table. One of the few games I managed to win so it really stuck in my memory haha.
Second story: I was playing vanilla Marines vs. Tau, and I had a unit of TH/SS Terminators charge his Kroot unit, he got to attack first since well, Thunder Hammers. He managed to inflict 5 wounds on my Terminators, no big deal right? I rolled all 1s. That squad was completely wiped out before inflicting a single wound on anybody during that game.
