These are heroes of a forgotten age who defeated the greatest evils of the time together.

Why would they suddenly become selfish and attempt to leave one of their own behind?

This I will give some serious thought... I had planned a "lesser of 2 evils" scenario in order to test our Paladin and Cleric...

Yes, it's deviate. I used something very like it once for a season-ender.
There are ways to get everyone out safely, but if you put a time limit on it (rising Lava) they may not think of them in time.

Another alternative for the above is to give them two doors. one will lead them to their destination in a short amount of time, with little danger, but they must leave somebody behind. the other, right next to the first ([1][2]), leads to a much longer journey, fraught with danger. the space between them is so small the application of a portable ram would break right through it.

Another way to challenge PCs is to take them out of there comfort zones, in my RT campaign at the moment the party is exploring a wrecked submarine for information on a NPC they're tracking. Its as you can imagine, underwater! Which means certain things don't work, moving about is harder, not to mention they're on a death world that's 70% water, so they'll be watching out for a few jungle obstacles/gribblies then it's diving time which will yeild some deep sea gribblies as well as the constant threat of if their suits are punctured they're out unless they surface in time. My idea for this was have them be stalked by a massive sea creature that swallows a couple of them, the PCs then have to kill it from the inside while the rest continue with the mission unsure of their outcome.

As for choices, mine are pretty basic (although i have snagged a few good ideas from this thread), such as, you see the figure of a man approaching through the gloom, he appears to be holding his gun up as he jogs along. So it's shoot first or be shot first, but if they shoot first it may have just been old man wilson coming to warn them about pressing danger! Or maybe it's said pressing danger.

In all seriousness, one method I plan on using in an upcoming game is to have some means of escape after the 'final battle', but there is not enough room for all, or only so much magic left in the runes to teleport everyone but one.

They will either discuss who should stay, or start scrambling for the exit.

Cozy cockpit of an Archer ARC-5S

Present them with overly obvious situations that aren't as obvious and clear cut as they turn out to be. Happened to us several times now. We were presented with a ticking timer on a space station so got the heck out, encountering a daemon along the way. Turns out the timer wasn't hooked up to a bomb. Two sessions later we come across what we thought were several rebel squadrons of escorts fighting it out with three Imperial cruisers, with the Imperials losing. We picked up an escaping Imperial shuttle and jumped out of the system. Contents of the shuttle: four trigger happy Navy officers and two Chaos sworn rogue psykers. Cue us going back to the system and apologizing to the "rebels" for mistaking them for the enemy.

Failing that, swamp them with hordes of some form of insignificant foe.
