Poor/bad sportsmanship tops the list. I'm playing the game to have fun and have a good time and you should be as well. When the dice gods decide to hand you 1's, just laugh it off and put your worst offending dice in "timeout" (the floor) instead of cursing or worse, throwing them in anger. And yes, that applies to the competitive environment. We are both there to win, but we should also be there to have a good time and my opponent's behavior dictates how much fun I am having.
I will also give an honorable mention to the GUOs who seemingly aren't aware of the... presence... their lack of proper hygiene creates. I live in a very hot climate and even with the AC on, when you cram a lot of people into an area the temperature rises and that's when the musk begins to fester. Worse still are the stores who save on a hefty electric bill by keeping the AC at 80 or even off altogether on 110 degree days. I'm all for the LGS saving money and continuing to operate but yikes it can get bad!

I haven't, but then again I've never had to. The one LGS that had a player with a hygiene problem would be told to leave the store by the owner on days when his... aroma, became too much for everyone in the store. At the other LGS I frequented the 40k players were all bathed and sufficiently protected by deodorants. It was the Role Playing group that used the store on Friday night that had the GUOs.

Well, when I worked at the local store it was in part my job to tell people off if they smelled highly offensive.
BO could simply be taken care of with a gentle word and offering a bit of febreeze. I find that alot of the time, most guys are unaware of their own odour, especially if they've just come from say a warm working environment or a sports event.
The outright worst ones imho are;
- Smokers. Filthy habbit and it makes you smell a million times worse than just pure body-stink. What's worse, is smokers who put the game on hold so they can go step outside and light-up for 10 minutes or so, and then drag that disgusting stench with them back into the gaming hall, then lean over the table and waft it right in your face.
Considering I'm not a smoker myself, having to play against a hardcore smokestack can easily be far, far worse than simply being near a GUO.
- Women spectators. I have no problem with guys bringing their significant other to watch, but for the love of feth, don't let them bath in an entire bottle of perfume! When I can chew on your fragence just by simply breathing, it's way too much. It's even worse when it turns into a poisonous gas attack and forces me to leave the room so I can simply breath clean air.

Greebo had spent an irritating two minutes in that box. Technically, a cat locked in a box may be alive or it may be dead. You never know until you look. In fact, the mere act of opening the box will determine the state of the cat, although in this case there were three determinate states the cat could be in: these being Alive, Dead, and Bloody Furious.

The worst type of player is the one who never stops talking about their army. During the course of the game, I want to have a good chat. I don't want to listen to how, why, and when you built your army. Fluff players are different, because they're just thinking of realism.

Body-stink doesn't phase me much honestly due to the vast number of years I spent playing competitive girls hockey. One season, our team got stuck with a girl who hadn't aired her goalie equipment out in over 3 years!
(don't even ask about washing it - suffice it to say, the sweat stains were turning dark brown )
The smell was so bad, we had to stick her in her own private change room.

People who forget the purpose is to have fun.
Our local group has a TFG who not only gets worked up over losing, he also gets worked up over facing certain armies (basically anything shooting-oriented), worked up over rule updates, worked up over the perceived failings of his own army, and gets worked up over any possible advice we try to give so that he might do a little bit better.
I mean, if you're not enjoying anything about the experience, why are you even bothering us with your presence?
My second choice would be people who randomly wander away from team games. Everyone loves that situation where we're resolving our turns, we turn around and go, "Where's X?"

I would say the douche-bag players, and the overly competive players, its fine to be competitive in tournaments, but I have seen my fair share of players who treat every game like their pride is on the line and just don't know how to have fun anymore, I also dislike the douche-bag overly competitive players, the kind of people who bring out their "A" game on a new player, it really hurts the community if new players are discouraged and leave.

Warhammer 40,000 is for better or worse a social hobby and it often contains all the problems that any social interaction might engender. People can often be narrow minded, opinionated, stubborn, and judgmental. This is simply a part of human nature.
The kind of player that irks me the most is the kind that forgets that other people are not exactly like them and forgets that the game is about fun. Some players like to play for fluff and others competition. Some like to be a little lax on the rules and others very strict. While some view painting as the most important part of the hobby and some people simply loathe it. It turns out people are different; how weird is that?
The point being that both sides need to loosen up a little bit, smile, laugh, and shake hands. At the end of the day if each person can walk away from a game with a degree of self respect then everyone has really won.
Aside: Something that bugs me too are the people that complain when others do not know the rules perfectly. I think you should remember that while 40k is not exactly complicated it does have a -lot- of special rules, some players are newer to the game than others, and physiology varies from person to person. The point being that some people are simply able to memorize more than others, some have more time to do so, and (heck) some have played so long that the editions literally begin to bleed together in their memory. If someone forgets a rule just politely point it out and move on. No need to be a jerk over it.

Two types of people urk me the most: 1) The "This makes sense" guy. Yes, when trying to decipher how a rule works you tend to go with what makes sense, but then theres people that try to add crap in because it makes sens (the big one being you can only see my arm so i get a 2+ cover....nothing says cover saves are additive except for vehicles which is ONLY a +1 to the normal cover if it EVER happens ... or has shrouded or stealth). Then they get all pissy when you quote rules at them denouncing their "makes sense" rule. Goes both ways too, people trying to ignore rules cuz it doesnt "make sense" - i had a GK player try to fight me when i charged his Dreadknight with a group of boyz and managed to keep my Nob out of combat for denying his challenge. That arguement went on for 15 minutes or so even though the BRB specifically said "Models that cannot strike or land attacks cannot be issued a challenge" and challenges are issued at the START of a combat, so where he ends up at the end means nothing. The annoying part was i wiffed all 4 my PK attacks after this long argument so it didnt do anything rofl. 2) Know-it-alls. This game is far too vast to know every single rule. I feel i know the BRB and my own codex quite well, but i dont flaunt BRB rules and challenge people on every single detail because i think theyre tweaking it slightly. You misinterpret or straight up overlook soooo many rules...you missed one...get over it. Not a game goes by where i dont look in the BRB to check something real quick. My last one was someone trying to make me lose +1attack and +1strength for charging through terrain - the BRB only says i lose initiative...i have a damn PK i could care less!! lol Im thankful i havent played with any of those table-flippers or the guys that complain about conversion models, though im sure they'd piss me off too. if someone said i couldnt use my Wazdakka or my Deffbrute(Hellbrute orkified) conversions because they werent legit models, i'd probably just leave without even trying to argue.

You won't believe the amount of times this has happened to me...
Had someone grab my SM codex out of my hand so roughly once that it pulled out a chunk of 6 or 8 pages. I looked at him mortified, expecting some quick follow up "oh my god I'm so sorry!". Instead he tossed the rest of my codex aside (to the ground no less) as what he was looking for was in the pages in his hand. He read it over as everyone else in the game store just stared at him. Finally he says "well I guess that's right", and drops the pages onto my codex (still on the ground) then turns to me with a plain look on his face waiting for me to continue my play. When I called him on the damage to my book, he just calmly picked it up, crammed the pages back in and said "it's fine".
Trying to stay calm myself, I calmly start to pack up my models. Everyone is still watching dead silent. As soon as he realized what I was doing he goes "so you forfeit huh? *chuckle*", and I just said nothing. Then he proceeded to FREAK out. Saying I had to tell him he won because I was giving up and quitting, and blaming it all on me because I was trying to cheat (to be honest, I don't even remember what the original rule was we were trying to look up). I kept saying nothing, then he snatched at one of my units I was trying to pack up, and I thought it was going to get physical, which was funny because I had about 60-80 pounds on this guy (he was about 5'8", late teens early 20s, and maybe 150lb wet. I on the other hand am 5'11" 220lb, some fat, mostly muscle as I worked construction at the time). But I kept my cool the whole time just kept cleaning my other units.
Finally all that was left for me to pack was the stuff he snatched. I asked him calmly for them (the first thing I said since he started freaking out) and he said he wouldn't give them back to me until I admitted he won. Not wanting to fight, with everyone still frozen watching, I said "ok you won". He throws the models at me! They were my metal vanguard vets (I know they suck but I love them and put a lot of conversion work on them). I desperately try to calmly pick up the pieces, as he starts talking trash about "oh god I owned you so hard noob". I don't even know what he was saying, I was just trying not to see red.
Then someone else in the store tells him he was losing and I was only packing up because he was throwing a tantrum. In response to that he starts yelling at her and me. When I ignore him and try to give her a look to do the same, HE FLIPS THE TABLE AT ME! Funny part was only his models were left on it. Finally the owner (a HUGE bald guy) grabs him and drags him out the door. He screams outside as the owner wont let him come back in, even to get his models. While we were picking up his models so he would go away (this taking about 20 minutes while he screamed outside at the owner) the cops show up, and while everyone cheered, they arrested him.
Never heard from him again, and the owner kept most of his army to cover the damages to the table, and gave me some of them.
And that's the story of why I have 6 fire warriors.

I have to admit, this is where I struggle Mostly it's just seeing all the enemy models on the table, and then seeing how you have one less Vindicator to take care of them. It's kind of hard to think of some ways to win, Even when I have the Rest of my army right behind that one tank.
Sometimes I've been known to surrender when it looks hopeless. It's a really bad habit.
There's always hope until the game ends on its own. Why, I once managed a draw with nothing but a single Space Marine Scout with a sniper rifle, and Saint Celestine left in my force by the end of the game. This was up against a mob of around 15 Boyz (down from starting with 20), 12 Lootas, a Big Mek with Shokk Attack Gun, and a pair of Killa Kanz in separate squadrons.
My Scout was parked behind a Defence Line in cover from everything else on the board, going to ground at every possible opportunity to get that nice, 2+ cover save. He was just barely holding onto the objective worth 4 points. The Boyz mob was trying to string out between 2 objectives, but Celestine charged into the middle of them to pull them away from the objectives with their mandatory combat pile-in. The Lootas and Big Mek were perched up high, the Killa Kanz on the ground, all blasting away with everything they had at my Scout for at least 2 turns solid after they'd slaughtered his squad-mates. He made every single cover save. After the game I calculated the odds of him making every one of those 2+ cover saves. Turned out to be roughly 6 in 1,000 (0.6%) if I remember right.
With all the secondary objectives considered, it came out as a draw. ^^ So never give up.

If it helps, they made it so you can only score one objective per troops choice now, so no more konga-scoring
