Guilliman will wake up with a gasp, his heart pounding. He'll look around and find himself not in a stasis-tank, but a soft bed, covered in rich satin blankets. Confused and terrified he'll stumble out of the bed, his super-human physique hitting the oakwood floors with enough force to make the cozy bedroom rumble. What's that? A hissing noise, in the distance. Guilliman will creep out of the room, crouching low, fists balled with tension. There, it's coming from the bathroom! Guilliman gently pushes the door open, and is met with a wall of steam. Squinting, the mighty Primarch wills his post-human gaze to pierce the bank. On the opposite end of the room he sees the shower, and there within... The Demi-God lets out a sigh of relief- he had forgotten where he was! Heart fluttering, he dashes forward, arms outstretched. "Oh, Matthew!" He cries. Matthew Ward, resplendently naked save for a single patch of soap around his nether-regions, has just enough time to turn around and address Guilliman before the Primarch is upon him. "My liege!" he utters in surprise. Guilliman barrels into the shower and embraces Ward, the warm water immediately soaking his nightgown through and through. "Oh, Matthew!" Guilliman weeps, burying his head in the writer's shoulder. "I had the most awful dream! The Imperium was beset on all sides by foes! The Galaxy was at war!" A shudder wracks Guilliman's body as he recalls the traumatic visions. "There was only war..." Matthew is taken aback by the suddenness of these circumstances, but he knows that it is his job as the Man to remain strong and comforting. He gently wraps his arms around Guilliman's waist. "It's okay, Roboute, it's okay." He says. He gingerly rocks the Primarch in his arms. "It was just a dream... just a dream." Guilliman sniffles. The camera pans around the two until Guilliman's back is to us, and we can clearly see Ward's face over his shoulder. Staring at the camera, his look of perplexion slowly morphs into a twisted, devious smile, and his eyes flash Necron-green. Vincent Price cackles in the background.

I predict that time itself will stop, that all actions cease the very moment the 41st millennium draws to a close. All the desperation of the moment will be locked up eternally at this critical point in history. Only the most powerful or knowledgeable entities and individuals will realize this tragic microcosm of history that consummates how all the architecture of grand designs and edifices of power mean nothing against the cataclysmic end of temporal progression. That is why the desperate like Ahriman and Abbadon futilely struggle against their own inescapable fate, launching last minute fights for relevance against the mere hope of turning against the clock that will never tick again. This is why the Farseers keep meddling in mortal affairs, their doomed race struggling to see the End Times when nothing will matter anymore as they will forever be preserved in a moment that will never advance again. Perhaps a few special beings will be able to still function in a time without time. Only the most powerful chronomancers of the Necron Dynasties and psykers of the Old One's creations could push through the thick and heavy present that will never lurch forth again. But their sheer force of will will never penetrate nor influence events again, as all around them they will see every tear shed, every bullet shot, every snuffed life end, but be unable to change any of it. For the lack of a better term, the universe will have no future beyond that one eternally imprisoned blip of time.

If it were to actually end there will probably be a separate ending for each faction so everyone gets to end as #1.
But story wise the Orks continue to multiply and advance, coming into contact with Tyranids more and more. The two fight epic battles that increase at an every faster rate. Meanwhile the Necrons get pounded by other Orks not currently fighting someone else. Awakening Tomb Worlds are over run, while any fully functioning ones fight both Orks and Tyranids. Eventually the greenskins (who already own much of the galaxy take over the mortal realm.
Then as always in the Eye of Terror forces battle each other for eternity as The Great Game plays on...

Orks: Gather under an incarnation of "The beast" and finally sweep through the galaxy killing human, xenos, heretic alike

Tyranid: The main fleet from the regions of dark space arrive. So numerable that no force is able to stop them.
Eldar: When every Eldar is dead. The god Ynead. God of Death will be born. Slaying the eldar's enemies.
Imperium: the god emperor dies but is reborn a god of order or reincarnated. All of the missing primarchs gather together to fight chaos and other xenos threats.
Chaos: the emperor dies. The astronomicon is out. Finnally chaos is able to corrupt, raid and kill unmercilessly. The entire galaxy is swept in corruption and filth as nearly everyone in the galaxy is dead. With most of humanity lost under the self infliction of chaos. The gods lose their very life essence and the galaxy goes quiet..
Necrons: The silent king finds an hidden device (or wakes up the remaining tomb worlds) that wakes all the tomb worlds across the galaxy. A menace as numerous as the imperium, uncomprehensibly advanced, as unrelenting as the orks, undying and incorruptible is released. The galaxy goes quiet with the glowing light of necrodermis.

It will eventually collide with the Andromeda galaxy forming an elliptical galaxy with very little star formation. The elderly stars of this new galaxy will slowly fizzle out over time until only black holes and red dwarfs are left. Then they will fizzle out and we get the heat death of the universe.

Tyranids eat everything is my guess. What we've seen thus far are just scouts, and the Tyranids as a whole have several galaxies of biomass behind them. Barring some unforeseen huge deus ex machina, I don't see anyone being able to withstand the tide.
But then... Several factions also have the means to get places where the 'Nids can't reach them. The Eldar, Dark Eldar, and Chaos can all hole up in places that the Tyranids (presumably) can't reach. I think that the Necrons would also be able to do this, if not with their own tech then with their access to the Webway. So, after the 'Nids eat everything and move on, whoever successfully managed to hide might come out, and live in the barren void of a galaxy that's left.

This is my feeling as well! Except for Eldar. Instead of everyone dying, the last Tear Of Isha is found and the God is born. In the Valedor book it is hinted at that they are very close.

The 5th god is already Malal/Malice, no? I only recently discovered it myself. Perhaps it would have a larger part to play in the end of the universe as we know it, aiding the destruction of both the Imperium and Chaos.
