The day before Games Day Atlanta I had just finished a unit of Eldar Rangers for GD entry. I was going to seal them then let them sit for the month. Shook the can and began to spray. It took me mili seconds to realize I had grabbed black primer instead of sealer but it was too late. In a fit of rage I threw the models into the woods behind my house, which to my knowledge still sit out there somewhere.
Worst part was I repainted a unit of rangers and when I went to seal them (used the right stuff this time  ). It was pollen season and several of the models fell off my sealing box and into the grass while still wet. They had weed seeds and pollen permanently glued to em. How realistic hehe.

Also in a game against my tyranid playing friend on the first day the new codex came out, he deep-strike pod'd 20 hormagaunts directly in my 3 obliterators faces. i then proceeded to barbeque 17 of them with twinlink flamers on my turn. ouch.

Late at night working on a conversion involving a daemonette torso, a witch elf arm, and an ungor spear, I realized that one spear arm I'd made for some reason wouldn't fit onto the body except in one weird position. I got mad at myself that I'd put the spear in her arm backwards and in trying to take it apart snapped the plastic spear just above the pin, taking a good couple cm out of it.
Then I realized later that I had intentionally put it that way, to put her in a pose different from the rest of her unit.

Was painting round some ork bases with graveyard earth ready for basing. I forgot to move them and the paint dried and stuck a large amount of them to the table, some of them came off thier bases when i tried to peel them off.

I would repeatedly forget to roll for my wyches’ combat drugs. You had to roll at the start of the game but before turns were taken, the rule book phrasing as "at the start of the battle". Back then I had a regular opponent who would insist that I missed my opportunity and half my games against him my wyches fought without chemical enhancement.

Assembling the Plastic/Metal Devestators was hellish I think they ended up saturated with the Super Glue. I glued the guns and arms to me more often than the model......That superglue brush is the greatest gift the emperor has ever given us!

The same happened to me with a killa kan. It took me a lot of time, and a lot of skin cells to assemble it. I can't wait for the new killa kan kits.

Ouch. Another incedent i had with a nightbringer is charging my howling banshees into one, only then realising they couldn't wound it  .

Store fantasy event where whoever gets most banners wins a $25 gift card... last turn I accidently pulled a banner out of a unit which caused me to lose a combat, get pursued and lose the battle... where as if I had kept it, I would have tied combat then won with the musician, kept my banner (and bsb banner that got over ran), and won the game... DOH

Seriously, Commisars are EASILY the most hardassed units in the game. Period. You cant argue with that at all. Specially the fluff. I mean they are HUMANS! Not super enhanced killing machines, not giant Orks or bugs or aliens. They are just really really REALLY scary men.

So in a 1500pt tournament game against Orks I had my entire Tau cadre on the back edge, as my opponent had a kanwall,30 boys and some Lootas. I figured Id drop some submunitions on him from my board edge while moving my forces to the flanks. His Lootas procedded to stun both hammerheads for the first 3 turns, basically stopping them from shooting until they are destroyed in assualt.... as the game comes to a close on turn 5 he shoots the Lootas at my Piranha and I remember that all my vehicles have D-pods...d-pods I had not been using the entire game...*Facepalm*

I occasionally forget to shoot with a unit, or forget to account for a USR or Special rule.
The worst modeling mistake I've made is accidentally slicing across a Space Marine's torso (Mk.8 armor :( ) I tried to make it look like battle damage, but I think I just made it much worse.

Too many in game to recount
Modeling: When I got my first GW box (Space Marine Tac squad) I happily assembled them one at a time. I was on the last step of putting on the heads, and the fifth head was different from the rest. It was a Corvus type or beakie head. I thought (in my 14 year old ignorance) that it was a mistake. So in my genius I cut of the beak to make him look like the rest. Needless to say, he now has a flat cut where his beak used to be.
To this day, I see that marine (painted and everything!) and cringe. He is usually assigned to be an extra wound on my devestator squad.

One of my D'oh! moments: Realizing that to possibly win the game, all I need to do is get my Imperial Guard Veterans over to the Objective. I figure, with a move and a decent run roll, they should be good to go. I forget to use Move! Move! Move! and they roll something abysmal. I'm just outside of the objective and I lose the game.
Also, forgetting about my Primaris Psyker sitting in a chimera attached to a Veteran squad for just about a whole game.
Another one: my brother and I are playing a team tournament. We don't play against Orks much and so we're thinking that we can deal with Nobz before they get to us. My brother had popped them out of their Battlewagon the turn prior and we hadn't had a chance to fire on them yet. My brother Lashes the Nobz closer to us (Eldar and Slaanesh, btw). We shoot everything we have at them and I don't think we killed a single one. They proceed to sweep-assault two of our units the next turn and then it turns into a bloodbath. Lesson learned. D'oh!
And lastly - I'm assuming my opponent said D'oh!, at least in his head. We were playing a tournament and we were finishing up. I didn't have an objective, but neither did he so, it was a tie. He moves his troops forward - very little I can do to stop him. The objective is one movement phase away for him. So, the next turn he moves in toward the little die sitting on a column and he settles in. The game ends and he smiles about his victory. Till he realizes that the objective he surrounded was the turn counter and not an objective at all. He was about 6" away from the real objective, so I secured the tie, since neither of us held an objective.

Don't want to turn this into a YMDC post, but as long as Typhus is within 2" of the unit, he joins them whether you like it or not (and whether you declare it or not). If he is not joining the unit, he must stay outside of 2" of the unit. Independent Characters, pg. 48 BGB. And that's all I'll say on that piece.

Playing Dark Angels against Sisters.
Ezekiel with tac sqd charges a penitant and he explodes it with first and only krak grenade attack.
Next game i tried it again, this time boldly using ezekiel... alone... with no inv save or backup of any kind. I blindly charge girlfriends p.engine, which she raises an eyebrow at, to which I confidently blurt out "oh i did it before! i'll do it AGAIN!".
...Sadly and not surprisingly, he misses with the krak grenade, and then again not surprisingly gets swatted like a gnat for his efforts.
GF laughs and continues her rampaging ways...
... I cried and went and made a coffee.

On another game of SH, my oponent revealed his brrodlord on a two blip and I let it pass not realising the error..

I used GreenStuff for the first time in my life about a week ago. I carefully read the instructions and rolled the blue and yellow parts together, appereantly not enough as it was still pretty yellow.
Yesterday, my buddy said to me that it is supposed to be entirely green when you model it to model. Then I responded "HOW THE HELL AM I SUPPOSED TO KNOW THAT GREENSTUFF IS SUPPOSED TO BE GREEN WHEN YOU MODEL IT". 10 people start to laugh at me and then I realised how stupid it sounded.

Several actually.
First, while building some hills out of expanded polystyrene, i was slicing it into shape with a hot knife. A hot knife is like a wire cutter, but a knife that plugs into the mains so it melts its way through the foam. Anyway, i was holding the foam in one hand, and dragging the knife with the other. It wasn't working very well, so i was using a lot of force. Force i neglected to reduce when it reached the edge of the foam... End result, one knife heated to 200C buried 3 inches into the fleshy bit between the thumb and index finger of my left hand. There was pain, blood and screaming.
Secondly, i was busy glueing some metal models together, when I found my superglue's nozzle had completely clogged up. No problem I though, in my genius i sliced open the foil container, and scooped out the glue with my sculpting tool. And all was well. Next day, i came back to glue some more, and forgot about the slice in the tube, and attempted to squeeze it onto the model. End result, a handfull of glue. I ran to the sink to try and wash it off, but somehow the glue was on my other hand too. End result, glued to the tap, and spending an agonising 10 minutes peeling myself free.
Thirdly, using my airbrush inside, i used a cardboard box to contain the spray to stop me repainting my walls. Except, in order to see what I was doing, i had to stick my head a fair distance inside the box, and inadvertently wound up inhaling a large amount of paint. My snot was blue for a week.

Except I took it in the palm and the blade went right through the other side.
Cauterized the wound though so there was feck all blood, I couldn't hold anything in my hand for about 4 months afterwards though...
I feel your pain man!
