Cherubs in Doom 3.
Bloody background growls pinkies made in Doom 1-2 always spooked me. And offcourse Cacodemon's hiss when they float into your face with that big grin of theirs.

That kind of reminds me of Ring for some reason.
The creepiest thing for me was probably under Karazhan with the Upsidedown Sinners in WoW. I hadn't read about it before, and had explored it by chance when WoW was relatively new. That was a creepy experience back then.

1.) The first time you see a licker in the police department!
2.) When you're in the police department, and you run into an interrogation room to find a chess piece key....and a licker jumps through the one-way mirror after you pick it up! Fething terrifying!!!
....needless to say, I was young when I played that game, and I thought lickers were the scariest things on the planet

The creepiest thing I've found in a game has to be a certain incident which happened while I was playing Stalker Call of Pripyat. To put this in context Stalker is fething creepy when it's daytime and the weather is good (ingame).
It was night, there was a thunderstorm in full swing and an emmision (massive radioactive explosion of instant death) had just started. The only cover I could get to was a particulary intimidating looking ruined factory. I only just got to it in time to shelter from the emmision. As I looked outside to see if it was dying down I noticed my visibilty metre was changing and I heard a sound behind me.
That is possibly the only time I've quit a game out of sheer terror.

Good call on that one, the Carrion carcass rushes and mr fleshbone (something like that) were creepy as hell. Took me ages to get through those levels.

Ooh actually, elite level difficulty on Dam for Timesplitters two. Those blooming zombies were NASTY. And the Notre Dame maidens. Girls who look like they are crying, then they look up and they look like something out of 40K.

Actually, although I haven't played it, the ending of Mother (or Mother 2, can't remember) is fething creepy, especially compared to the rest of the game. Most of it is a happy band of friends taking on hordes of pesky bug thingies, and then you get to the boss, which looks like a womb drawn by H.R. Giger, and then turns into a red skull faced fetus, and THEN, to defeat it you go back in time and ABORT it in the womb. Doubly creepy in a game called Mother.
What makes it even worse is that it was inspired by a horrible experience the creator had where he walked into the wrong play at a theatre, in a scene where a woman is murdered...And the poor sap thought it was a RAPE scene.
Argh.
For that matter, coming back to my original post, I actually found Portal 2 quite creepy. Especially GLaDOS's blood curdling electronic scream midway through the game. I can never look at a modem the same way again.

The atmosphere and the screams that paralyzed Link creeped me out sufficiently that it took a good few weeks to get past that particular level...

Silent Hill (the first one) was probably the first game I felt honestly disturbed while playing. As far as more recent ones, Call of Chtulu: DCoE was pretty terrifying, as was the Penumbra series. Honestly, I think they were worse than Amnesia. It was good, but there were parts in the first Penumbra where I just had to walk away for a while because it was getting to be too much.

The headcrab zombies are probably the creepiest i have encountered myself...
That said i tend to start shooting things as soon as i start getting creeped out which can ruin the atmosphere in many games...

Which silent hill game (if it's silent hill. I'm assuming it is) has you fight a monster in I think a toilet/bathroom. It's arms reach the floor and it hasn't got hands just a kind of hammer instead. Fighting that.
The ps2 (I think, long time ago) CoC game, shadows over innsmouth I think. Most of that game  Especially when you just have to run from the townspeople
Oh Timsplitters 3's under the mansion level. "Ooh Princess! Dinner time!"
And I gotta agree with 2's notre dame level. Still find it funny when you see quazimodo with a shotgun.
Finally: Majora's mask is creepy as hell looking back. You're wearing the face of deceased people and it hurts to put it on (He screams when it happens) Not to mention stuff like the Ufos and how certain people act arround midnight on the last day. Some creepy stuff in that game.
