I always go 1:1 with Bleach Bone and White. If that isnt light enough for you try 2:1 White and Bleach Bone

I use a combination of NoQuestionzAsked's and Brymm's methods; all the steps that Brymm detailed, except I highlight with a 1:1 mix of skull white & bleached bone instead of straight skull white (and I don't wait for the 'eavy metal invitation :p)
