This is my 6th Edition take on her:
Wow, that took a couple hours, referencing C:SoB 2nd Edition, BRB 2nd Edition, C: SoB 5th Edition, and BRB 6th Edition. Aside from some language clean up, and preferred word choice of GW's, I think it came out nicely. I did have to tweak the Sceptre and Irresistible Charge a bit to make them work better with the fluff of the mace itself and 6th Edition assaults/challenges.
I tried to keep her as close as I could to her original rules. I dropped her down to T3 from T5 to fit current SoB/human stats. For the Sceptre and Irresistible Charge, I took both the fluff parts and the rules part and mixed them together to better fit 6th Edition and balance. If one wanted to stick more to her original RAW, drop Unwieldy from the Sceptre and have the Charge halve WS as opposed to the pseudo-Concussive effect.

That's a lot of work you put in -- thank you. Four suggestions:
1) Someone who knows better than me should look over costing -- I find that very tricky for SCs.
2) "Irresistible charge" seems, ironically, like the kind of thing that would slow down play because it's fairly complex -- and since it only comes up rarely, it might be easy to forget to do it at all. I'd suggest giving her a Hammer of Wrath attack (ie one executed before the usual initiative sequence) at the Mace's strength.
Furious Charge (+1 S) and/or Rage (+1 A) are charge bonuses well worth considering too.
3) Given her fluff -- braining a Hive Tyrant and all that -- either she or the Mace should have Monster Hunter. I'd argue for the Mace itself, because...
4) ... I'd like to see the Mace of Pradexes as an Ecclesiarchy relic, one that Pradexes must use if she's in your army but another Canoness could use if you set your game after her death. My fluff explanation would be that, after her heroic last stand to protect the evacuation, the planet underwent Exterminatus; when Pradexes' grieving Sisters returned to the scoured planet to place a monument at the site of her death, they found her mace miraculously intact amidst the devastation and brought it back. Now it is a revered relic of the Sisterhood, given to only the most worthy and trusted champions embarked on urgent missions against Tyranids, Daemons, and other Monstrous Creatures.

No, she's too over powered
Her original stats were Ws 5, Bs 5, S 3, T 3, W 3, I 4, A 3, Ld 10, Sv 2+ 130 points
I can see her getting a +1 boost in Ws, Bs, and I, though
Deny the With on a 4+? Sounds like Space wolf territory, will you settle for a 5+? which is still better than what the sisters have now.
On the other hand, the sisters could use a Sisters of Silence type character
your idea of Sacred Rights, i don't think the sisters need help with acts of faith rolls (+1 for HQ, +1, for unit leader, +1 for casualties, is a roll of a 2+) the sisters need more faith points, though
sydney has great ideas too.
something i found searching the net: old stats from 3rd edition, want to update her for 6th?

Unless she had stats from prior to the 2nd edition C: SoB, then no, those aren't her stats. Her stats were in that codex:

Did you read what Purity Seals did from C: SoB from 2nd Edition? C:SoB 2nd Edition wrote:...If the character is targeted by a psychic attack, they will be unaffected on a D6 roll of 4+.

Scared Rites were essentially the SoB AoF, prior to them being AoF and St. Praxedes added 3 to that roll. Again, changing to how AoF work now and what boost she gave then, adding 2 instead of the normal 1 did not seem unreasonable and jives best with what she did back in 2nd. Look at the current rules and stat line for St. Celestine, what St. Praxedes brings to the table and for how much seems pretty damn reasonable. She's still T 3 and no EW, and she won't be coming back like St. Celestine. Hell, she didn't even have an Invul save and I only gave her one through the Shield of Faith as opposed to just giving her a 4++ for no good reason.

Special rules: Independent character, Stubborn, Furious charge, Inspirational leader, Titanic strike, Eternal warrior, Puritanical Zeal, Adamantium will, Faithful (The Fury)
Warlord Trait: Know the Witch The Warlord and her unit the 'Hatred' and 'Preferred enemy' special rules.
Inspirational leader: Grants all friendly units from 'Codex: Sisters of battle' within 12'' the 'Stubborn' special rule.
Puritanical Zeal: An army where she is your Warlord may re-roll a single set of charge dice once per turn.

Will just have to take a look
she looks good, though my advise is the act of faith could be worded better, for example, when to roll for the faith test and if this is two separate acts of faith or not
