I re-roll any dropped dice, and I clarify this early on.

And I'm consistent - succeed or fail, I re-roll ALL dropped dice.

If I've dropped dice to the floor or a roll sees it careen off the edge, then it's always been an understood that it gets rerolled.

If i drop a dice from the handfull without rolling it, I immediately ask my opponent, who invariably says reroll.

We consider this a standard in the group I game with.

I found a good solution...especially if your playing on a board with lots of terrain or really cluttered up with models
roll all dice into a box lid or something, then every dice that lands in there counts, because you cant really claim that you accidentally dropped it there.
plus, it saves knocking loads of stuff over and possibly moving things around
