Codex in hand and notepad open, start setting up pre-set builds that I like (eg. a vet squad with carapace and 3 plasma guns right as the top so I can cut/paste without calculating the point cost several times or memorizing anything). Frequently I will break up my army into "sections" that don't necessarily fight together, but simply happen to be the same number of units. Eg. If I have 6 vet squads and 3 tanks I will make 3 sections of 2 vet squads in a tank so I can just cut/paste the cost to more quickly calculate it. It takes me like 5 minutes to get a basic list started so I can fine tune it. I find it faster than hopping around an excel document adding and deleting entries until I get everything right.

Word Processor, I usually have a rough idea in my head of the army list i'm going to build and using Word Processor just allows me to refine that and display it in a manner I can reference too.

I always start on paper unless its Mordheim, in which cause i use the actual Mordheim sheets.
For things like tournaments or a list that I use a lot I make it look fancier on Microsoft Word using a Table.

pen and paper is good for disposable army lists that you experiment with. For something I plan on using longer, I use a computer, so I can make variations on it.
