Simple question: Do you have a model that continously fails and what does it do/not do?
For me it would have to be my BT plasma gunner. Every game I play (with one exception) he has rolled one to hit,failed his save and killed himself in the gets hot.
But what about you?

My Ork Lootas. I can't seem to make them shoot enough dakka or hit for that sakes. They have so far never killed anyone in a game.
And i still take them with me? Why i don't know...

Though my own units don't really have too many problems (although my KFF Mek has his moments) my friend has a Tau Hammerhead that rolls an inordinate amount of ones for hits or wounds.
This has gotten to the point I've ignored it several times because I know it's not going to hit a damn thing.

It's not a model, but the one thing that always fails me is the roll on the vehicle damage table. Every time I always easily penetrate a LRBT with my Lascannon predator I roll a 1 for crew shaken. Then the broken tank gets to fish itself out of the bin.

My tyranid hormagaunts. Last time I played them my 20 strong squad got beaten in close combat then ran down by a 10 man guardsmen squad (I had the charge too). Nice goin guys

Well, It would have to be my leman russ demolisher. The reason why is every game it either get's destroyed firt or second turn when it's outta range. Or it just get shaken and stunned results the whole game so it can't do anything, ever! Even when I finally get one shot off, it scattered back onto itself and exploded and killed 5 guys next to it. It;s fething awful!

Zoanthropes. I have had one game where they successfully killed a tank or two, but in every other game I have fielded them (around 6-8 games...) they have either failed psychic tests, missed entirely, or simply been shot and failed their 3++. There was one time that they killed an entire space marine. That was great.
I now use them only for synapse in very small games, because I know that they can't do anything else.

A toss up between my Storm Troopers and HWTs. Doesn't matter what I do with the HWTs, I never kill a thing. The STs always make an awesomely dynamic entry and get butchered by some unit that is like 20pts.

My Biker Warboss, after he was finally finished being painted, did nothing but fail for a long while.
First game after being painted, he rides over a ravine to get to the enemy army. Fails his dangerous terrain test, takes a wound. Enemy turn, he gets shot by a storm bolter, takes a wound. My turn again, he assaults an enemy rhino (after his deffkopta escort failed to even touch it with their TL rokkits), blows it up, and dies in the explosion.
In another game, he rockets across the map into the enemy Space Marine lines, only to lose combat and die to a 5-man unit of tac marines.
Finally, in a game against the Tau, he makes it into combat with a unit of Firewarriors and their Devilfish. He makes one (autohit) attack against the Devilfish, and the rest against the Firewarriors. He misses the Devilfish, and whiffs all but one hit agains the fire warriors. The single hit fails to wound, and then the fire warriors inflict one wound on him. He fails his leadership test, runs, and then gets SWEPT BY FIREWARRIORS.
Thankfully though, he's been slowly making a bit of a comback, having almost solo'd half of a combined Tyranid/Ork force in a 2000 pts doubles game, and killing over 400 points worth of Death Cult Assassins and Grey Knights in his most recent forray. Hopefully his success is not fleeting.

Probably my Wolf Guard Squad.
I kit them out with PWs, PFs, WCs, and THs. And all they do is take wounds for Arjac Rockfist. They have never made it out of a combat, and their Land Raider has never survived a game.

So far, I have played 6 games with my Shokk Attack Gun. In 4 of them, he has blown himself and his squad up in 3 turns or less, in 1 of them he shot himself into CC with pink horrors on turn 2, and in 1 he did what he was gakking supposed to.

I'm surprised it lasts that long! They're pretty easy to wreck.
My consistently under performing units would have to be my three Vindicators in my Iron Warriors army. Turn 1 sees one get wrecked, turn 2 sees one get immobilized, and by turn 3 they've either all been wrecked, all lost their guns, or all immobilized facing the wrong direction or something. When they actually fire, they almost always miss, fail to wound, or in some cases land on my own guys. Anything that hits their armor rolls straight sixes with startling regularity, and even against armies with limited long range firepower, every shot hits and penetrates on these things. They're like a magnet for bad luck.

Same for me. If they weren't the only unit I had with a 3++ save then I wouldn't bother with them. Last game, threw 2 11s for psychic tests, and a couple of 1s on the pen role for the lance attack. In 4 turns they lived 3 of them immobilised one predator - for 180 points. I get more use out of the mycetic spore.

A specific one of my Aspiring Sorcerers for my Thousand Sons used to fail so regularly that he earned the nickname "Kenny". He'd either blow his own head off with Perils of the Warp, fail to hit or wound anything, or explode into a Greater Daemon which would then completely fail to do anything as well.
He's been getting better lately though! I suspect he's been earning experience and levelling up behind my back or something.

My bigmek. Yesterday he killed his first model ever. Except for my original AOBR orks, he is the oldest model I own.

7 Ork facts people always get wrong:
Ragnar did not win against Thrakka, but suffered two crushing defeats within a few days of each other.
A lasgun is powerful enough to sever an ork's appendage or head in a single, well aimed shot.
Orks meks have a better understanding of electrics and mechanics than most Tech Priests.
Orks actually do not think that purple makes them harder to see. The joke was made canon by Alex Stewart's Caphias Cain books.
Gharkull Blackfang did not even come close to killing the emperor.
Orks can be corrupted by chaos, but few of them have any interest in what chaos offers.
